- To assist you to identify your social care needs and to obtain a free assessment from your Local Authority
- To contact appropriate providers and services who are able to provide the care required. This can range from: a couple of hours per week: either at home or outreach support in the local community; shopping attending health appointments or local groups etc: a more complex care package; Live In Care; or a residential or nursing home.
- To provide you with a clear informative report, which includes details of the provider or services charges, capacity, availability or vacancies and their Care Quality Commission (CQC) report status.
